Referral Information

For Hearthstone of Minnesota’s services to be most effective, youth must be able to function in a non-secure, residential environment and to benefit from a structured stable family care setting. They must be able to engage in an appropriate educational and/or vocational plan within the local school and community. 

Youth are typically referred through their local county social services or corrections department/agency. The placing agency is responsible for making the initial referral and for submitting referral information which may include the following: previous service plans; treatment and placement history; previous treatment plans and discharge summaries; educational and medical records; psychological or psychiatric evaluations; court documents; screening committee documents; and screening tool results.

Hearthstone is also able to accept private referrals.
